Any ideas for treating hands with very painful contractures?

Hello.  I am new to this forum.  I care for my Mum who has advanced Lewy Body Dementia.  She has very painful hands that are permanently clenched.  We have bought palm protectors to stop the nails digging into her palm.  Her hands are so painful when touched that she shouts out and refuses to let us do anything to them, hence her nails are in an appalling state and we are really struggling to keep her hands clean.  She was prescribed Baclofen to relax the muscles but as she seemed to get unwell just after starting the treatment we were advised to stop.

Any ideas as to how to treat this problem would be welcome
