CHC Checklist

Hi, I’m new to the Forum and looking for some advice. My husband was diagnosed with Parkinson’s 11 years ago and as you can understand his health has deteriorated during this time. At the moment he’s in hospital and I’m trying to find out what help/care will be available should we have to go down a different route than the one we were previously on.

I’ve been told by someone at Parkinson’s UK that on their website there is a CHC Checklist - I’ve looked and looked some more and just can’t find it. Does anyone here know where the link is? Or, can anyone provide the link on this message?

The CHC (Continuing Healthcare) Checklist is part of the NHS assessment process, and we do have information about it on our website.
