Hi For continued health care do i need Docs/Hospital notes

Hi Myself and my Sister and Brother are going to apply for continued health care for my Mother, and I just wanted to know do you need to apply for the Doctors/Hospital records?
If so any tips/help would be well appreciated

Hi David,

For hospital records you need to apply to the records department of your local NHS trust.
For GP records apply to your surgery.

There are forms to fill in and a bit of a wait but no charge, where I live anyway.

Go on the caretobediferent.co.uk site.
Read everything.
You will be guided through the extremely detailed process.
First demand a checklist.
Then hopefully you will move onto a DST meeting.
You will have to fully understand all the domains and collect masses of evidence.
The site and its helpline are invaluable.

Be prepared for an exhausting time in which you will be tempted to give up.
