Parkinson’s with Severe Head Drop

HI All,
I came from Hong Kong and live in Glasgow now.
My wife was diagnosed Parkinson over ten years. Until 2019, neurologist prescribed Sinemet 25/100 mg, three times daily . However, after just one day of taking the medication , she developed sudden and persistent head drop , accompanied by muscle tightness and motor disorder . Her head has since tilted downward at nearly 90 degrees, severely affecting her mobility and independence.
Despite multiple referrals — including to orthopaedics, physiotherapy, and botulinum injections — no treatment has succeeded in alleviating the condition.
Do anyone also has head drop situation? Any advice, resources, or referrals you could offer would mean a great deal to us.

I’m really sorry to hear this; it must be very difficult for you both. My husband’s head drops suddenly sometimes and he seems to go to sleep instantly with it - even if we are having lunch out…I have got used to it, but your wife’s sounds different. My husband has been to physio for his neck and he didn’t get much help, but I have just resorted to giving him a hot water bottle that goes round his neck which he says helps relieve the muscles (but it doesn’t stop his head dropping). Your wife’s sounds very severe and I hope you can find something that helps.

The exact same thing happened to me (within one day) and I eventually came off the levodopa for 3 weeks guided by the neurologist but it didn’t resolve so I wondered if it was mirtazapine 15mg which I started a few days before the levodopa but I haven’t managed to come off that so I still don’t know…

Hi, I suffer with a head drop and my necks twisted to the left. I have had it 4-5 years. I’ve tried physio , massage and Botox injections. Nothing has helped. I had to give up on the injections because they were causing choking. My head is always looking down and can’t look people in the eyes. My neck gets very tired if I’m trying to lookup for to long. I haven’t met any other PDP who experience this. I find I fall over a lot and I think this is one of the reasons why. I can’t link it to any particular medication.
