Sleep attacks

Hi. My mom was diagnosed with Parkinson 2 years ago, although had symptoms since around 2020/21.

They put her on lepadova 3 times a day and 2mg patch. This has been working although slowness and gait was more of an issue for her than the tremors.

In January she started getting these sleep attacks where she was suddenly start slurring her speech, saying random things - her mouth would drop open and she would start breathing/dribbling, eventually she would drop into sleep and then the severe twitching/jerking and repetitive movements would start. Along with very loud noises. She also talks a lot during this time but we cannot understand what she is saying. It’s almost like she is acting out her dreams?

She can then switch back to normal after.m and doesn’t recall she’s doing it and says she doesn’t feel tired. These are happening more and more but the Parkinson’s nurse doesn’t know how bad they get as she hasn’t seen one first hand. My question is, have any others experienced this too? Is the medication now not right for her or is this just normal progression of Parkinson’s? Her feet are also extremely swollen so we are worried that could be something along side the Parkinson’s that might also be causing these sleep attacks.

Sorry for the waffling post, we are really stuffing as a family at the moment to come to terms with this and want to try our best to get her comfortable. It just seems like very fast progression which we was not expecting so soon as it has now taken away all her independence.

This could be a form of REM-sleep Behaviour Disorder (a common symptom of PD). I had this but it is now well treated with 1mg of clonazepam just before I go to bed.

I know that I have started talking in random bouts of sleep too since being on my medication. I know exactly what you are saying about not getting it across to the PN because I feel the same. I don’t know what the law says about privacy, but would it be possible to take a short video of her on your phone camera to show the nurse what is happening? Like I say, you’d need to get advice on the privacy of something like that but it might be a help.
