Hi Weedy and Westby

I'm glad you saved all this info Weedy, it is too valuable to lose.

perhaps we could put together a generic form to help all future claimants?

people think they are defrauding the G-men and feel guilty over claiming something that they are fully entitled to.
they tend not to describe how they feel on a bad day.

its probably not legal, but hey, whose looking?

J:grin:
Good idea I'm willing to chip in. I like the term G men it sums them up perfectly.Perhaps to be pc we should refer to them as G people.
YOU HAVE A PROGRESSIVE DEGENERATIVE NEUROLOGICAL DISEASE.
YOU ARE ENTITLED TO WHAT IS AFTER ALL, A PITTANCE.
BUT ITS A PITTANCE THAT WE NEED.

MAKE SURE YOU CLAIM.
IT WILL BE BACK DATED FROM THE DATE YOUR FORM IS RECIEVED.
IT WILL TAKE BETWEEN 8-13 WEEKS, SO BE PATIENT.
IT IS NOT MEANS TESTED, SO IF YOU ARE LUCKY ENOUGH TO HAVE A MILL OR TWO STASHED, YOU CAN STILL CLAIM.

HERE IS A COPY OF MY SUCCESSFUL CLAIM.
I GOT THE HIGHER MOBILITY AND MIDDLE OF CARE.

Yes

MY BALANCE HAS REALLY DETERIOTED
OVER THIS LAST 9 MONTHS AND I STUMBLE
QUITE REGULARLY OR BUMP INTO THINGS.
MY SHINS ARE COVERED IN CUTS AND
BRUISES. THE MEDICATIONS ARE THE CAUSE
OF THE DIZZY SPELLS AND OCCASSIONAL
SLEEP ATTACKS.

ANYTIME BUT MAINLY WITHIN AN HOUR OF
TAKING MY MEDICATIONS.OR IF I AM GOING
UPSTAIRS WHICH I TRY TO KEEP TO A MINIMUM
NOW. I HAVE FALLEN OFFCHAIRS BECAUSE
I TURNED TOO QUICKLY.

WHEN I STUMBLE OR FALL, I WOULD NEED
SOMEONE TO ASSIST ME TO GET UP. ITS GOT
WORSE RECENTLY BECAUSE I DONT HAVE
THE STRENGTH ANY MORE TO PULL MYSELF
UP. I TRY TO GET TO MY KNEES AND THEN
ASK FOR HELP.

I FALL AT LEAST ONCE A DAY, UNLESS I STAY
IN BED,AND I STUMBLE AND SHUFFLE ON A
REGULAR BASIS.ITS HARD TO PUT A FIGURE
ON IT, BUT ABOUT 5 TO 10 TIMES A DAY.

I HAVE PRACTICALLY STOPPED GOING UPSTAIRS
NOW EXCEPT TO CHANGE CLOTHES. I
HAVE FOR THE LAST 3/6 MONTHS BEEN
SLEEPING ON THE SOFA. THE STAIRS ARE
THE MAIN PROBLEM ESPECIALLY COMING
DOWN. I AM ALWAYS AFRAID I MIGHT FALL
DOWN THEM. I TEND TO STAY IN THE LIVING
ROOM WITH AN OCCASSIONAL TRIP TO THE
TOILET OR KITCHEN

WE HAVEN,T THE MONEY TO ADAPT ANYTHING,
BUT I,AM LOOKING TO CHANGE THE
DINNING ROOM INTO A BEDROOM. I WOULD
ALSO LIKE TO GET EXTRA RAILS PUT UP
ABOUT THE HOUSE , ESPECIALLY THE BATHROOM,
WHICH I DOON,T USE VERY OFTEN
BECAUSE ITS UPSTAIRS PLUS I FIND IT HARD
TO WASH (DYSKENSIAS KICKS IN AND RESTRICTS

I TEND NOT TO USE THE BED ANYMORE AND
SLEEP ON THE SOFA IN THE DOWNSTAIRS
LIVING ROOM.MAINLY DUE TO THE STAIRS
BUT ALSO BECAUSE I SUFFER BADLY WITH
INSOMNIA.MY WIFE TRIES HARD TO GET ME
TO GO TO BED BUT IT SUITS ME BETTER
DOWN STAIRS

BY SLEEPING ON THE SOFA ITS EASIER TO
GET ON AND OFF BECAUSE ITS LOWER. I
ROLL OFF AND THEN PULL MYSELF UP BY
THE SOFAS EDGE. IF I WAS IN BED IT WOULD
BE MORE DIFFICULT AND WOULD ALSO DISTURB
MY WIFES SLEEP WHICH I TRY MY BEST
NOT TO DO.IF I DID RETURN TO SLEEP UPSTAIRS
I WOULD REQUIRE RAILS TO ASSIST

OF THE RESTLESS LEGS COUPLED
WITH INSOMNIA I WAS CONSTANTLY ASKING
MY WIFE TO GIVE ME A PUSH WHICH IS UNFAIR
ON HER. BY USING THE SOFA, I CAN USE
THE BACK PART FOR LEVERAGE TO ROLL OR
EXIT

CONCERNING WASHING AND BATHING

when my wife can coerse me into the bath , i try to
cover as much as i can myself, but my wife has to
wash my hair, my back and neck and my feet and
lower legs. drying myself is also a problem. i
cant co-ordinate to dry the awkward areas. i can
do most places my right hand can reach and my
wife does the rest. when i have a bath , its normally
last thing at night so that i dont have to get
dressed again.
I also gives my wife no respite. she is more a captive
to this disease than i am. if it affects my wife
badly ,it then in turn hurts me.
most of my friends i rarely see, and i cant blame
them. we have nothing in common except memmories.
i miss my work and collegues and something to get
up for in the mornings.

Disability Living Allowance and
other benefits

This note contains two separate pieces of information. The first part is for you and is about your claim for
Disability Living Allowance. The second part is for your carer, if you have one, and gives information about
Carer's Allowance.

Please read this information and bring it to the attention of your carer, if you have one. To print this page,
select "Print this page".

For you -your benefit could be affected if someone claims Carer's Allowance for providing care for
you

If your claim to Disability Living Allowance is successful, you may receive an extra amount called the severe
disability premium (or additional amount for severe disability in Pension Credit) paid as part of

ā€¢ Income Support
ā€¢ Pension Credit
ā€¢ Housing Benefit or Council Tax Benefit
If someone claims Carer's Allowance for providing care to you and he or she is are paid Carer's Allowance
you may not be able to receive this extra amount. For more information about this, contact the office dealing
with these benefits.

For your carer -about Carer's Allowance

If you are caring for someone who is intending to claim Disability Living Allowance, you may wish to make a
claim for Carer's Allowance at the same time. You should not wait until the person you are caring for
receives a decision on their claim for Disability Living Allowance. If you do not make a claim at the same time
you could lose benefit. Ask for the claim pack DS700.

Carer's Allowance and other social security benefits

Payment of some other social security benefits affects payment of Carer's Allowance. This means if you are
receiving another benefit you may not be paid Carer's Allowance or any payment may be reduced. However
even if you cannot be paid, being entitled to Carer's Allowance means that you may be able to get an extra
amount paid with income-based Jobseeker's Allowance, Income Support, Pension Credit, Housing Benefit or
Council Tax Benefit

How to obtain help and advice and claim Carer's Allowance

ā€¢ You can obtain information from the Department for Work and Pensions website (www.dwp.gov.uk)
and claim on-line by selecting 'Disabled People and Carers' and then following the links to the Carer's
Allowance e-service.
ā€¢ You can write to Carer's Allowance Unit, Palatine House, Lancaster Road, Preston, PR1 1HB or phone
01253 856123 (text phone 01772 899489 for people with hearing or speech difficulties) or email to
[email protected]
ā€¢ You can ring the Benefit Enquiry Line (BEL) on 0800 88 22 00. This is a confidential telephone service
for people with disabilities, their representatives and their carers.
ā€¢ Your local social security office and many local associations such as Citizens Advice can provide claim
forms and help with completion.
The claim pack and the website contain notes which explain Carer's Allowance in more detail. Claim forms

It's also a good idea to tell your gp that you are applying for DLA before you apply. When mine suggested that we ran through my difficulties and care needs she was amazed by how little she understood about them.As she said at the time if the G men had contacted her she would have totally under estimated my needs. Don't assume that the professionals understand or know the problems that PD brings.
hi guys. just to let you know

when i applied for DLA 6 years ago, the whole 'claiming any benefit was foreign to me.
following some advice, i contacted a 'Revenues officer' at my local council.Told her i was unable to complete all the forms, and she visited, filled in all forms and even got carers allowance for my wife.

most importantly, there is someone who will come to you and do it all for you. you have an incurable neurological diseaes, dont be affraid to claim

martin
hi i get hi rate dla but you no when you recieve it it says it runs out in2010 i got it in 2004 does this mean you have to apply again because if thats so its mad everybody knows pd gets worse
Hi Alison

I agree that in 2010, like you , I will have to re apply for DLA, but when that time comes, follow my advice; contact your 'Welfare Benefits ADvisor' at your local council.
Ask them for a home visit to assist in the completion of your application, as you are unable to fill in forms.
They will oblige.
The DWP told me today that, even chronic illnesses such as pd need reviewing, simply to make sure you are getting the top award, however, they also explained that it gives opportunity to review your individual case....for example, your circumstances may have changed because you spend one week a month in a home (just an example)
They stressed that if your condition remains unchanged then so will your entitlement.

hope this helps

martin
Hi All

I forgot to mention that the 'welfare Benefits Advisor' who came to see me, also got my wife full carers allowance, and sorted out details that entitled us to Income support..
It seems that there are allot of 'Benefits' available (depending on circumstances) but they don't tell you...you have to find out what they are....that's why i strongly urge you to use your local council's Benefits Advisor..

martin
thankyou martin that is very helpful do you get high rate dla.
yes Ali i get high rate care and mobility.

martin
this is an actual copy of my DLA form.

it was when i was known as Jupiter

I've changed the numbers obviously but it will give you a guide.

REMEMBER....YOU FILL OUT THESE FORMS AS THOUGH ITS YOUR WORSE DAY.

YOU DONT HAVE GOOD DAYS .....JUST BETTER ONES THAN PREVIOUS

YOU ONLY HAVE BETTER DAYS BECAUSE OF YOUR MEDICATION.

YOU ARE ENTITLED TO THIS BENEFIT......REMEMBER, AS YOUR WORSE DAY
Pokermid, I have to say that I found your filled-in form very moving as a snapshot of what it is to have Parkinson's, especially the parts about your job and your friends. You are one courageous person and I think it's brilliant the way you keep your humour, reading your posts often makes me laugh. Never mind the DLA application form, what about some more brilliant writing?!
Thank you Tasmin, thats very kind of you
Good one Pokermid, a helpful aid and can I add to photocopy your application for future reference, should you need to apply for a higher level in any category in the future.
pokermid,my dla gets looked at again in 20010,even if people say i get high rate now,can things change and go down,if the dr got ur meds under control.even if u need the help still.
Apparently that can happen.................

However, can any meds make such a difference that you actually go down a level on DLA? We wish it were so.

I am wondering right now if we should apply for an upgrade from middle rate care to higher rate care and from lower mobility to higher.

The hoops you have to jump through makes me wary of applying, as though we were begging, in some way.

So, would value your opinions. OH now needs help dressing most days and cannnot do much for himself. Shakes all through the night keeping me awake, shuffles around and rests a lot. Can walk places close by but does not drive. Should we ask for higher rate DLA on both counts??

Seeing neuro on 28th as I am so concerned at his recent deterioratin.
Hi Benji, yes I definitely think you should apply for the higher rate. I am sorry that your OH is suffering that much.
yes benji,i agree with jem, contact dla and apply.
Thanks Jem and alij.

Will have a look at the forms and dig out my photocopy of the last set from years ago.
Hi Benji,
Good luck with the forms.I've been waiting since October to hear back about claim for son.Reading the forms it does say please inform them of any changes.Hope you manage to get higher rate, sorry to hear OH is getting worse, hope neuro appointment can help.
Take care Dot xxx
